New Patient Registration

We are able to register patients eligible for NHS care within our catchment area.

Our practice area is outlined below (e.g. areas of IP1, Bramford, Great Blakenham, Sproughton, Claydon) but if in doubt, our reception staff can give precise details.

We cannot register patients outside our area (e.g. IP2 IP3 IP4 IP5 postcodes) and if you change address to an area outside the practice boundary you will be required to re-register with a local practice serving these areas.

Details of the registration process and documentation required are available at reception.

You will need to complete a New Patient Information Questionnaire. You do not need proof of address or immigration status, ID or an NHS number. Patients aged (over five) will be offered an appointment for a New Patient Health Check with the Healthcare assistant ( HCA ) within three months of registration.

For children (under five) who are also required to have an appointment, please bring in the Child Health red book to enable our administration team to retain on file the most up to date records of childhood immunisation details.


Next of Kin/Carer/Support Worker Details

Please provide reception with their details in order that they can be entered on your medical record.


Practice Area

This practice accepts patients living to the western side of Ipswich as well as in the villages of Claydon, Bramford, Sproughton, Great Blakenham, Henley, and Akenham.
